We commend Ben-Horin et al1 for their pivotal randomized trial evaluating video capsule endoscopy (VCE)–guided treat-to-target strategies in quiescent Crohn’s disease (CD). The study demonstrates a significant reduction in clinical flares with proactive VCE-driven interventions, advancing precision medicine in inflammatory bowel disease. However, several critical questions warrant further discussion to contextualize these findings and guide future research.
