We read with great interest the landmark study by Tavaglione et al1 recently published in Gastroenterology, which provides a simple and scalable biomarker panel to identify steatotic liver disease (SLD) with excessive alcohol use. The authors should be commended for addressing a critical unmet need, particularly given the limited availability of phosphatidylethanol (PEth) testing and the well-recognized challenge of underreporting of alcohol intake. The integration of PEth into the outcome definition and the demonstration of consistent performance across independent cohorts represent important strengths that support the clinical relevance of this work.
