We must make a serious commitment
\nto increase financial resources and provide better analytics for real world
\nevidence\/real time data in support of public health.<\/h4>\n

Public health has been underfunded
\nfor decades. That neglect has had a profound impact since the COVID-19 pandemic
\nhas taken hold, and awakened policy makers and thought leaders to the need for
\nmore investment.<\/p>\n

Consider the statistics: The U.S.
\nspends about $3.6 trillion each year on health but less
\nthan 3%<\/a> of that amount on public health and prevention. A 2020
\nForbes report<\/a> likewise pointed out that \u201cFrom the late 1960s to the 2010s,
\nthe federal share of total health expenditure for public health dropped from 45
\npercent to 15 percent.\u201d This relative indifference to public health is partly
\nresponsible for the nation\u2019s mixed response to the SARS-CoV-2 pandemic. A recent
\nMcKinsey & Company<\/a> analysis concluded: \u201cGovernment leaders remain
\nfocused on navigating the current crisis, but making smart investments now can
\nboth enhance the ongoing COVID-19 response and strengthen public-health systems
\nto reduce the chance of future pandemics. Investments in public health and
\nother public goods are sorely undervalued; investments in preventive measures,
\nwhose success is invisible, even more so.\u201d<\/p>\n

Among the other \u201cpublic goods\u201d that
\nrequire more investment is population health management and analytics. Although
\nexperts continue to debate the differences between public health and population
\nhealth, most are unimportant. For our purposes, population health refers to the
\nstatus of a specific group of individuals, whether they reside in a specific
\ncity, state, or country. Public health usually casts a wider net, concerned
\nabout the status of the entire population. Managing the health of these
\nsubgroups requires an analytical approach that can take into account a long
\nlist of variables, including social determinants of health (SDoH), the content
\nof their medical records, and much more. SDoH data from Change Health care, for
\ninstance, has demonstrated that economic stability index (ESI) is a strong
\npredictor of health care utilization. ESI is a cluster model that uses market
\nbehavior and financial attitudes o group individuals into one of 30 categories,
\nwith category 1 representing persons most likely to be economically stable and
\ncategory 30 least likely to be stable. The figure, which links race, ESI and health
\ncare utilization in Kentucky, suggests that Blacks\/African Americans are far
\nless likely to be economically stable (category 1). The same analysis found
\nthat Blacks\/African Americans were almost twice as likely to use the ED
\ncompared to Whites (30.5% vs 18.1%). A growing number of health care
\norganizations are starting to see the value of such population health metrics
\nand are incorporating these statistics into their decision making.<\/p>\n

Among the valuable sources of data
\nthat can inform population health are patient surveys, clinical registries, and
\nEHRs. Several traditional analytics tools are available to extract actionable
\ninsights from these data sources, including logistic regression. Over the
\ndecades, several major studies have also generated risk scoring systems to
\nimprove public health. The Framingham heart health risk score has been used for
\nmany years to assess the likelihood of developing cardiovascular disease over a
\n10-year period. Because the scoring system can help predict the onset of heart
\ndisease, it can also serve as a useful tool in creating population-based
\npreventive programs to reduce that risk. The tool requires patients to provide
\ntheir age, gender, smoking status, total cholesterol, HDL cholesterol, systolic
\nblood pressure, and whether they are taking antihypertensive medication. The
\nAmerican Diabetes Association has developed its own risk scoring method to
\nassess the likelihood of type 2 diabetes in the population. The tool takes into
\naccount age, gender, history of gestational diabetes, physical activity level,
\nfamily history of diabetes, hypertension, height and weight. Another analytics
\nmethodology that has value in population health is the LACE Index. The acronym
\nstands for length of stay, acuity of admission, Charlson comorbidity index (CCI),
\nand number of emergency department visits in the preceding 6 months. More
\nrecently, there are several AI-based analytic tools currently being used to
\nimprove population health. A review of ML-related
\nanalytic methods<\/a> found that neural networks based algorithms are the most
\ncommonly used (41%) in this context, compared to 25.5% for support vector
\nmachines, and 21% for random forest modeling.<\/p>\n

There is no way of knowing how the
\nworld would have coped with COVID-19 had policy makers fully invested in public
\nand population health programs and analytics. But there\u2019s little doubt that we\u2019ll
\nall fare much better during the next health crisis if we put more time, energy,
\nand resources into these initiatives.<\/p>\n<\/div>\n\n","protected":false},"excerpt":{"rendered":"
