It was winter and she had been wearing her little jumpers and singlets quite a bit.\u00a0 Assuming her fine coat must be rubbing on the fabric we took her jumpers off during the day and only popped them on during the cold nights.<\/p>\n<div id=\"attachment_5408\" style=\"width: 310px\" class=\"wp-caption aligncenter\" wp_automatic_readability=\"32\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-5408\" class=\"size-medium wp-image-5408\" src=\"http:\/\/pressplaypets.com\/wp-content\/uploads\/2025\/11\/My-Dog-Has-Cushings-Disease-3-300x262.jpg\" alt=\"\" width=\"300\" height=\"262\" srcset=\"https:\/\/pressplaypets.com\/wp-content\/uploads\/2025\/11\/My-Dog-Has-Cushings-Disease-3-300x262.jpg 300w, https:\/\/pressplaypets.com\/wp-content\/uploads\/2025\/11\/My-Dog-Has-Cushings-Disease-3-768x671.jpg 768w, https:\/\/pressplaypets.com\/wp-content\/uploads\/2025\/11\/My-Dog-Has-Cushings-Disease-3-1024x895.jpg 1024w, https:\/\/pressplaypets.com\/wp-content\/uploads\/2025\/11\/My-Dog-Has-Cushings-Disease-3.jpg 1482w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\"\/><\/p>\n<p id=\"caption-attachment-5408\" class=\"wp-caption-text\">Amber\u2019s Coat Thinned Out<\/p>\n<\/div>\n<p>Amber was due for her senior full blood test.\u00a0 A small patch of fur on her neck was clipped and several months later the section clipped still had not fully regrow.\u00a0 The vet had also mentioned on several visits how her skin is so fine and fragile.<\/p>\n<div id=\"attachment_5409\" style=\"width: 310px\" class=\"wp-caption aligncenter\" wp_automatic_readability=\"32\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-5409\" class=\"size-medium wp-image-5409\" src=\"http:\/\/pressplaypets.com\/wp-content\/uploads\/2025\/11\/My-Dog-Has-Cushings-Disease-4-300x255.jpg\" alt=\"\" width=\"300\" height=\"255\" srcset=\"https:\/\/pressplaypets.com\/wp-content\/uploads\/2025\/11\/My-Dog-Has-Cushings-Disease-4-300x255.jpg 300w, https:\/\/pressplaypets.com\/wp-content\/uploads\/2025\/11\/My-Dog-Has-Cushings-Disease-4-768x652.jpg 768w, https:\/\/pressplaypets.com\/wp-content\/uploads\/2025\/11\/My-Dog-Has-Cushings-Disease-4-1024x869.jpg 1024w, https:\/\/pressplaypets.com\/wp-content\/uploads\/2025\/11\/My-Dog-Has-Cushings-Disease-4.jpg 1208w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\"\/><\/p>\n<p id=\"caption-attachment-5409\" class=\"wp-caption-text\">Clipped And Bruised After Bloods<\/p>\n<\/div>\n<p>During this time Amber had become very picky with her food, she would get super excited about breaky or dinner than when offered, turn her nose up and walk away.\u00a0 We tried everything, slow cooked beef brisket and roast chicken to entice her to eat.\u00a0 She started losing weight.\u00a0 We had regular vet checks trying to work out the source of her loss of appetite. We kept a food diary for a month to show our regular vet; Amber was now only eating around 50g of food a day!\u00a0 Not convinced Amber was being \u201cpicky\u201d it was highly recommended we have her looked at by an Internal Medicine Specialist at the Animal Referral Hospital (ARH); our vet was suspicious Amber may have Cushing\u2019s Disease.<\/p>\n<p>Ambers appointment specialist appointment consisted of a thorough initial consult, examination along with some tests including bloods and ultrasound.\u00a0 They made an unexpected discovery, Amber was suffering from a gallbladder bile disease <em>(we were told quite common in older dogs and often never discovered unless an ultrasound happens to be done)<\/em> and although not showing all of the \u201cclassic\u201d signs of Cushing\u2019s Disease, the specialist too was suspicious of the condition.<\/p>\n<p>First step was for Amber to undergo an <strong>ACTH Stimulation (Adrenocorticotropic Hormone) test<\/strong>, to see where her Cortisol levels were sitting.\u00a0 Blood was taken to look at her Cortisol base line; a synthetic ACTH injection given and one hour later another blood sample is taken to see if the adrenals produce enough cortisol in response to the stimulation.\u00a0 Insufficient response suggests Addison\u2019s disease while a spiked response may indicate Cushing\u2019s disease.\u00a0 <em>(Note: Coincidently my previous Papillon \u201cCorey\u201d lived with Addison\u2019s disease nearly his entire life. I was thinking \u201cOK if its Addison\u2019s again at least I know how to deal with that!\u201d)<\/em><\/p>\n<p><strong>The results:<\/strong><\/p>\n<p><strong>TIME (hours)\u00a0 \u00a0 CORTISOL (nmol\/L)<\/strong><br \/><strong>0\u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0439 (30-100)<\/strong><br \/><strong>1\u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 806<\/strong><\/p>\n<p><em>NORMAL DOG<\/em><br \/><em>Post-ACTH cortisol:\u00a0\u00a0\u00a0\u00a0\u00a0 200 \u2013 380 nmol\/L<\/em><\/p>\n<p><strong>DIAGNOSIS:<\/strong> Suggestive of Cushing\u2019s Disease.\u00a0 The ACTH stimulation test result by itself should not be considered diagnostic, but must be interpreted considering history, clinical signs and other laboratory date.\u00a0 Sick or stressed dogs may yield a false positive.<\/p>\n<p>As Amber had lost weight <em>(she is a tiny dog even before losing some weight)<\/em> it was safe to presume she was feeling nausea and stress from weight loss and the gallbladder bile.\u00a0 We agreed it was best to take a conservative two-pronged approach to her treatment, with both drugs to treat each condition is often not well tolerated and can cause vomiting and diarrhoea.<\/p>\n<p><strong>Step 1:<\/strong> Commence treatment only for the gallbladder first.\u00a0 Once treated for around a month we are hopeful her appetite will return and in turn her weight will increase.\u00a0 She should feel a bit if not a lot better.\u00a0 We also started Amber on a drug called \u201cMirtazapine\u201d, commonly used to treat depression in humans, <em>that just so happens to have a helpful side effect that makes you feel hungry!\u00a0<\/em><\/p>\n<p>Amber responded well to the medication and showed no signs of vomiting or diarrhoea.\u00a0 One month in and we were able to get her back to a healthy and stable weight.<\/p>\n<p><strong>Step 2.<\/strong> Now it was time to do the <strong>LDDS (Low-Dose Dexamethasone Suppression) test<\/strong>.\u00a0 A crucial tool that assists in confirming a true Cushing\u2019s diagnosis.\u00a0 Unlike the ACTH one-hour test, this is over an eight-hour period.<\/p>\n<p>TEST ONE: The first sample of blood is taken before the injection of dexamethasone.<br \/>TEST TWO: The second sample is taken four hours after the dexamethasone injection<br \/>TEST THREE: The third sample is taken eight hours after the dexamethasone injection.<\/p>\n<p>To keep Amber stress levels to a minimum, rather than leaving Amber at the clinic for the day, we requested to bring her home between each test so she could relax and have a sleep.<\/p>\n<p><strong>The results:<\/strong><\/p>\n<p><strong>TIME (hours) CORTISOL (nmol\/L)<\/strong><br \/><strong>0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 92 (30-100)<\/strong><br \/><strong>3 or 4\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a033<\/strong><br \/><strong>8\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 195<\/strong><\/p>\n<p><em>FINDINGS<\/em><br \/><em>4- or 8-hour post-dexamethasone cortisol:<\/em><br \/><em>&gt; 28 nmol\/L\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 consistent with hyperadrenocorticism<\/em><br \/><em>20-28 nmol\/L\u00a0\u00a0\u00a0 equivocal<\/em><\/p>\n<p><strong>Amber has Cushing\u2019s Disease.<\/strong><\/p>\n<p>The treatment for Cushing\u2019s Disease is a drug call \u201cTrilostane\u201d, we were already very concerned about Amber\u2019s weight, especially if she did not tolerate this drug well. It could be a complete disaster; she could start losing weight again or even worse end up on ED with terrible vomiting and diarrhoea.<\/p>\n<p>Again, under our specialist\u2019s advice it was decided to commence treatment conservatively.<\/p>\n<p>Amber\u2019s treatment plan:<\/p>\n<p><strong>AM \u2013 Treat Cushing\u2019s: \u00a0\u00a0<\/strong><br \/><em>Amber has breakfast<\/em><br \/>Trilostane 0.2ml<\/p>\n<p><strong>PM \u2013 Treat Gallbladder:<\/strong><br \/>Mirtazapine <em>(increase appetite)<\/em><br \/>Ondansetron Odt-WGR Wafer <em>(nausea)<\/em><br \/><em>1 \u00bd hours later \u2013 Amber has dinner<\/em><br \/>Ursodeoxycholic 0.5ml<\/p>\n<p>We also have another drug on hand called \u201cCerenia\u201d.\u00a0 This is quite useful if we suspect any nausea or diarrhoea.\u00a0 Amber has responded so well to all her medications we have not had to offer Cerenia very often.\u00a0 When we do it has been for suspected nausea alone.<\/p>\n<p><strong>Two-week Review:<\/strong> Amber is back to have an ACTH test to determine if the Trilostane is working to reduce her Cortisol levels.<\/p>\n<p><strong>The results:<\/strong><\/p>\n<p><strong>TIME (hours) CORTISOL (nmol\/L)<\/strong><br \/><strong>0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 138<\/strong><br \/><strong>1\u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0272<\/strong><\/p>\n<p><em>NORMAL DOG<\/em><br \/><em>Post-ACTH cortisol:\u00a0\u00a0\u00a0\u00a0\u00a0 165 \u2013 320 nmol\/L<\/em><\/p>\n<p>Amber\u2019s specialist is very happy with the results so far!\u00a0 Before having the test, we could already see a positive change in Amber\u2019s demeanour; we were hopeful the results would be optimistic.<\/p>\n<div id=\"attachment_5407\" style=\"width: 235px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-5407\" class=\"size-medium wp-image-5407\" src=\"http:\/\/pressplaypets.com\/wp-content\/uploads\/2025\/11\/My-Dog-Has-Cushings-Disease-2-225x300.jpg\" alt=\"\" width=\"225\" height=\"300\" srcset=\"https:\/\/pressplaypets.com\/wp-content\/uploads\/2025\/11\/My-Dog-Has-Cushings-Disease-2-225x300.jpg 225w, https:\/\/pressplaypets.com\/wp-content\/uploads\/2025\/11\/My-Dog-Has-Cushings-Disease-2-768x1024.jpg 768w\" sizes=\"auto, (max-width: 225px) 100vw, 225px\"\/><\/p>\n<p id=\"caption-attachment-5407\" class=\"wp-caption-text\">My Funny Little Girl!<\/p>\n<\/div>\n<p>She <strong>IS<\/strong> eating <em>(not woofing down her food, which honestly, she has never done, preferring a back massage, but I guess who wouldn\u2019t right?).<\/em> Regardless she is eating breaky and dinner <em>(mostly)<\/em> and she is happy and playful again. Which is such a positive sign and a <strong>HUGE<\/strong> relief.<\/p>\n<p>The plan for Amber now is to continue seeing her specialist on a regular basis (monthly to start with, stretching this out depending on her progress).\u00a0 ACTH tests will continue to be part of her on going plan and medication tweaked, as necessary.<\/p>\n<p>I will be honest with you; to say we were DEVASTATED with Ambers diagnosis of both conditions is AN UNDERSTATEMENT.
