That\u2019s why we\u2014an undergraduate student and emergency physician\u2014co-founded the <a href=\"https:\/\/steppingstrong.bwh.harvard.edu\/naloxone-administration-program\/#naloxone_education\" target=\"_blank\" aria-label=\"Link opens in new window (Naloxone Education Initiative)\">Naloxone Education Initiative<\/a>, with the mission of expanding opioid and naloxone education into high schools.\u00a0<\/p>\n<p>As a part of this initiative, we developed a curriculum that has been endorsed by fourteen harm reduction and health organizations in Massachusetts and across the United States. To date, we have delivered this training to more than 500 students, with plans to reach more than 1,000 others in the upcoming months.\u00a0<\/p>\n<p>Our twenty-minute training includes an overview of the opioid crisis, as well as instruction in how to recognize the signs of an opioid overdose and how to respond using naloxone, a medication that can immediately reverse an opioid overdose. We also tell students where they can find naloxone in their communities and explain <a href=\"https:\/\/malegislature.gov\/Laws\/GeneralLaws\/Partiii\/Titleiv\/Chapter258c\/Section13\" target=\"_blank\" aria-label=\"Link opens in new window (medical amnesty policies)\">medical amnesty policies<\/a> which protect people who administer naloxone or call 911 to save someone\u2019s life during an overdose.\u00a0<\/p>\n<p><a href=\"https:\/\/drugabusestatistics.org\/drug-overdose-deaths\/\" target=\"_blank\" aria-label=\"Link opens in new window (Since 1999)\">Since 1999<\/a>, more than one million people in the United States have <a href=\"https:\/\/wonder.cdc.gov\/\" target=\"_blank\" aria-label=\"Link opens in new window (died)\">died<\/a> from drug overdoses, with opioids <a href=\"https:\/\/drugabusestatistics.org\/drug-overdose-deaths\/\" target=\"_blank\" aria-label=\"Link opens in new window (accounting for 72 percent)\">accounting for 72 percent<\/a>\u00a0of these deaths. One of the approaches to addressing this crisis is a set of public health strategies known as <a href=\"https:\/\/www.samhsa.gov\/substance-use\/harm-reduction\" target=\"_blank\" aria-label=\"Link opens in new window (harm reduction)\">harm reduction<\/a>, which focuses on keeping individuals safe while using substances and providing non-judgmental care, rather than requiring abstinence.<\/p>\n<p>A significant breakthrough for overdose prevention occurred in <a href=\"https:\/\/www.fda.gov\/news-events\/press-announcements\/fda-approves-second-over-counter-naloxone-nasal-spray-product#:~:text=The%20agency%20approved%20the%20first,opportunities%20for%20nonmedical%20use%2C%20accidental\" target=\"_blank\" aria-label=\"Link opens in new window (March 2023)\">March 2023<\/a>, when certain formulations of naloxone became available as an over-the-counter nasal spray. While this greatly increased the accessibility of the drug, mere availability does not ensure that it is used, due in large part to the stigma surrounding opioid overdose.\u00a0<\/p>\n<p>The prevalence of this stigma among youth, especially, has contributed to an alarming rise in adolescent overdose deaths. In 2022, an average of twenty-two high school-age adolescents <a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMp2312084?query=featured_secondary\" target=\"_blank\" aria-label=\"Link opens in new window (died each week)\">died each week<\/a> from overdoses, primarily due to fentanyl-laced counterfeit pills. A bystander was <a href=\"https:\/\/www.cdc.gov\/mmwr\/volumes\/71\/wr\/mm7150a2.htm?s_cid=mm7150a2_w\" target=\"_blank\" aria-label=\"Link opens in new window (present)\">present<\/a> during 67 percent of teenage overdose deaths between 2019 and 2021, but Naloxone was only administered in 30 percent of cases.<\/p>\n<p>In looking at this data, we realized that countless young lives could be saved if youth are equipped with the knowledge and skills to administer naloxone. But while naloxone training programs have been <a href=\"https:\/\/www.cdc.gov\/mmwr\/preview\/mmwrhtml\/mm6423a2.htm\" target=\"_blank\" aria-label=\"Link opens in new window (piloted)\">piloted<\/a> in various community settings, most schools <a href=\"https:\/\/www.npr.org\/2023\/10\/05\/1203320228\/naloxone-can-save-students-lives-but-not-every-school-has-it\" target=\"_blank\" aria-label=\"Link opens in new window (have yet to incorporate)\">have yet to incorporate<\/a> this curriculum.<\/p>\n<p>Through our initiative, we have reached classrooms across Massachusetts, including public, private, and charter schools in both urban and rural communities, with varying demographics and overdose rates. After each training, we collect feedback from students on our presentation.\u00a0<\/p>\n<p>The feedback so far has been overwhelmingly positive. Students have reported finding the training informative, interesting, and easy to understand. Those who had previously witnessed an overdose and were unsure how to respond said they felt more confident in their ability to take action. And many students now feel empowered to get involved in advocacy efforts at their school to help make this education standard.\u00a0<\/p>\n<p>Our curriculum was developed in collaboration with<a href=\"https:\/\/steppingstrong.bwh.harvard.edu\/\" target=\"_blank\" aria-label=\"Link opens in new window ( Stepping Strong Center for Trauma Innovation)\"> Stepping Strong Center for Trauma Innovation<\/a>,<a href=\"https:\/\/www.naloxoneproject.com\/\" target=\"_blank\" aria-label=\"Link opens in new window ( The Naloxone Project)\"> The Naloxone Project<\/a> and community partners and instructors who provided valuable feedback. The<a href=\"https:\/\/drugpolicy.org\/resource\/safety-first\/\" target=\"_blank\" aria-label=\"Link opens in new window ( Drug Policy Alliance)\"> Drug Policy Alliance<\/a> and<a href=\"https:\/\/med.nyu.edu\/departments-institutes\/emergency-medicine\/divisions\/emergency-medicine-community-outreach-partnerships\/community-education-initiatives\/prevention-education-partnership\" target=\"_blank\" aria-label=\"Link opens in new window ( Prevention Education Partnership)\"> Prevention Education Partnership<\/a> have created similar curricula surrounding harm reduction.\u00a0<\/p>\n<p>We also helped craft a Massachusetts bill, <a href=\"https:\/\/malegislature.gov\/Bills\/194\/HD1593\" target=\"_blank\" aria-label=\"Link opens in new window (H.530)\">H.530<\/a>\/<a href=\"https:\/\/malegislature.gov\/Bills\/194\/S382\" target=\"_blank\" aria-label=\"Link opens in new window (S.382)\">S.382<\/a>, which will require opioid and naloxone education in the health curriculum across all Massachusetts high schools. Similar bills have previously passed in\u00a0<a href=\"https:\/\/capitol.texas.gov\/tlodocs\/88R\/billtext\/pdf\/HB03908I.pdf\" target=\"_blank\" aria-label=\"Link opens in new window (Texas)\">Texas<\/a> and<a href=\"https:\/\/legacylis.virginia.gov\/cgi-bin\/legp604.exe?241+ful+SB726S1\" target=\"_blank\" aria-label=\"Link opens in new window ( Virginia)\"> Virginia<\/a>. We hope Massachusetts will soon join them in ensuring that every child throughout the state will graduate from high school knowing how to reverse an opioid overdose.\u00a0<\/p>\n<p>Crafting policy, we have learned, is not a top-down process, but rather one that requires deep engagement with affected communities and grassroots advocacy with broader policy initiatives. The stigma surrounding opioid use makes it challenging to initiate conversations about naloxone.
