Vance and House Speaker Mike Johnson, seated behind the President, smirked, shook their heads, and laughed. A chorus of Republican Congresspeople responded in turn.\u00a0<\/p>\n<p>As a nurse and public health researcher who has witnessed firsthand the value of U.S.-sponsored programs in Lesotho\u2014programs now threatened by Trump\u2019s <a href=\"https:\/\/www.theguardian.com\/us-news\/2025\/mar\/05\/trump-supreme-court-foreign-aid\" target=\"_blank\" aria-label=\"Link opens in new window (blanket cuts to foreign aid)\">blanket cuts to foreign aid<\/a>\u2014I did not see anything to laugh about.<\/p>\n<p>I first visited Lesotho in September 2016 as a twenty-two-year-old Fulbright grantee. I was drawn to Lesotho by its rugged beauty, rich culture, and unfortunate distinction as the nation with the <a href=\"https:\/\/data.worldbank.org\/indicator\/SP.DYN.LE00.IN?most_recent_value_desc=false\" target=\"_blank\" aria-label=\"Link opens in new window (world&#x2019;s lowest life expectancy)\">world\u2019s lowest life expectancy<\/a>.\u00a0<\/p>\n<p>I came with little: two bags, one pre-existing social connection, and near-zero fluency in the local language. Still, the people of Lesotho welcomed me with open arms. Strangers walked me to work, gently correcting my pronunciation as I clumsily expanded my\u00a0<em>Sesotho\u00a0<\/em>vocabulary. Coworkers indulged my curiosity, teased me for my coffee consumption, and asked twice daily if I was eating enough.\u00a0<\/p>\n<p>Friends invited me to pick-up soccer games, weddings, and family reunions. There were some hard moments, but far more often, I felt gratitude for the life-altering impacts of America\u2019s\u00a0then-seventy-year-old <a href=\"https:\/\/www.fulbrightprogram.org\/about\/#:~:text=The%20Fulbright%20Program%20Is%20Established,framework%20for%20the%20Fulbright%20Program.\" target=\"_blank\" aria-label=\"Link opens in new window (investment)\">investment<\/a>\u00a0in fostering mutual understanding through the Fulbright program.\u00a0<\/p>\n<p>Through my nine-month Fulbright and a subsequent year-long stint with\u00a0<a href=\"https:\/\/www.pih.org\/country\/lesotho\" target=\"_blank\" aria-label=\"Link opens in new window (Partners In Health)\">Partners In Health<\/a>, I traveled to thirty health centers to help examine how changes to leadership, infrastructure, staffing, and supply chains impacted health care access across Lesotho. Within remote clinics <a href=\"https:\/\/geneva.usmission.gov\/2013\/11\/04\/health-care-lesotho\/\" target=\"_blank\" aria-label=\"Link opens in new window (financed)\">financed<\/a> by U.S. grants, I spoke with nurses, <a href=\"https:\/\/icap.columbia.edu\/what-we-do\/our-expertise\/human-resources-for-health\/nepi\/\" target=\"_blank\" aria-label=\"Link opens in new window (educated)\">educated<\/a>\u00a0and\u00a0<a href=\"https:\/\/www.usaid.gov\/global-health\/health-areas\/hiv-and-aids\/information-center\/blog\/learning-from-lesotho\" target=\"_blank\" aria-label=\"Link opens in new window (employed)\">employed<\/a>\u00a0through the President\u2019s Emergency Plan For AIDS Relief, who served as the only source of health care in a three-hour walking radius.\u00a0<\/p>\n<p>I spoke with village health workers who\u2014for $1 per day\u2014led educational talks, conducted health screenings, and accompanied sick patients over mountains and rivers to treatment. I spoke with patients who described how sustained investments in clinics, nurses, and essential medicines made health care worth seeking.<\/p>\n<p>\u201cWhen I became pregnant,\u201d\u00a0one mother told me,\u00a0\u201cI was not sure when to start coming to the health center. My three prior deliveries were at home. This time, I came to the health center because otherwise, there would be nobody to manage complications.\u201d<\/p>\n<p>Lesotho has the world\u2019s second-highest\u00a0<a href=\"https:\/\/www.who.int\/data\/gho\/data\/indicators\/indicator-details\/GHO\/prevalence-of-hiv-among-adults-aged-15-to-49-(-)\" target=\"_blank\" aria-label=\"Link opens in new window (prevalence of HIV)\">prevalence of HIV<\/a>,\u00a0second-highest <a href=\"https:\/\/data.worldbank.org\/indicator\/SH.TBS.INCD?most_recent_value_desc=true\" target=\"_blank\" aria-label=\"Link opens in new window (tuberculosis incidence)\">tuberculosis incidence<\/a>, and\u00a0ninth-highest <a href=\"https:\/\/data.worldbank.org\/indicator\/SH.STA.MMRT?most_recent_value_desc=true\" target=\"_blank\" aria-label=\"Link opens in new window (maternal mortality ratio.)\">maternal mortality ratio.<\/a> Its health care workers save lives. Through sun and snow, they deliver babies, administer vaccines, and treat conditions from heart failure to tuberculosis. In just fifteen years, their efforts have helped Lesotho achieve\u00a0massive decreases in <a href=\"https:\/\/www.unaids.org\/en\/regionscountries\/countries\/lesotho\" target=\"_blank\" aria-label=\"Link opens in new window (HIV incidence)\">HIV incidence<\/a>\u00a0and a ten-year increase in\u00a0<a href=\"https:\/\/data.who.int\/countries\/426\" target=\"_blank\" aria-label=\"Link opens in new window (life expectancy)\">life expectancy<\/a>.<\/p>\n<p>Americans should be proud of our contributions to this effort. Since it was <a href=\"https:\/\/www.npr.org\/sections\/goatsandsoda\/2023\/02\/28\/1159415936\/george-w-bushs-anti-hiv-program-is-hailed-as-amazing-and-still-crucial-at-20\" target=\"_blank\" aria-label=\"Link opens in new window (established)\">established<\/a> by George W. Bush in 2003, the President\u2019s Emergency Plan For AIDS Relief (PEPFAR) has <a href=\"https:\/\/www.kff.org\/global-health-policy\/fact-sheet\/the-u-s-presidents-emergency-plan-for-aids-relief-pepfar\/\" target=\"_blank\" aria-label=\"Link opens in new window (saved)\">saved<\/a> more than\u00a0twenty million lives. Yet in 2024, across\u00a0<a href=\"https:\/\/www.state.gov\/where-we-work-pepfar\" target=\"_blank\" aria-label=\"Link opens in new window (fifty-five countries)\">fifty-five countries<\/a> with PEPFAR-funded programs, the initiative accounted for\u00a0<a href=\"https:\/\/www.brookings.edu\/articles\/pepfar-delivers-outsized-returns-it-deserves-more-funding\/\" target=\"_blank\" aria-label=\"Link opens in new window (less than 0.1 percent)\">less than 0.1 percent<\/a>\u00a0of U.S. spending.\u00a0<\/p>\n<p>The new administration\u2019s cuts imperil this progress. While a March 5 <a href=\"https:\/\/www.theguardian.com\/us-news\/2025\/mar\/05\/trump-supreme-court-foreign-aid\" target=\"_blank\" aria-label=\"Link opens in new window (Supreme Court ruling)\">Supreme Court ruling<\/a> ordered the administration to release appropriated funds, Trump\u2019s sweeping ninety-day <a href=\"https:\/\/www.whitehouse.gov\/presidential-actions\/2025\/01\/reevaluating-and-realigning-united-states-foreign-aid\/\" target=\"_blank\" aria-label=\"Link opens in new window (aid suspension)\">aid suspension<\/a> and <a href=\"https:\/\/www.npr.org\/sections\/goats-and-soda\/2025\/02\/25\/g-s1-50701\/usaid-freeze-judge\" target=\"_blank\" aria-label=\"Link opens in new window (defiance)\">defiance<\/a> of prior court orders have already caused harm.\u00a0<\/p>\n<p>In Lesotho, where nearly one in four adults <a href=\"https:\/\/phia.icap.columbia.edu\/wp-content\/uploads\/2021\/08\/53059_14_LePHIA_Summary-sheet_with-coat-of-arms_WEB_v2.pdf\" target=\"_blank\" aria-label=\"Link opens in new window (lives with HIV)\">lives with HIV<\/a>, funding cuts mean widespread <a href=\"https:\/\/apnews.com\/article\/trump-aid-usaid-pepfar-africa-hiv-396263cd9e7281af7d543d3c68fa465a\" target=\"_blank\" aria-label=\"Link opens in new window (staffing layoffs)\">staffing layoffs<\/a> and <a href=\"https:\/\/pedaids.org\/press-release\/lifesaving-services-terminated-for-10000-children-and-10000-pregnant-women\/\" target=\"_blank\" aria-label=\"Link opens in new window (terminated treatment)\">terminated treatment<\/a>. They mean <a href=\"https:\/\/www.state.gov\/wp-content\/uploads\/2024\/12\/DREAMS-Fact-Sheet_WAD-2024_FINAL.pdf\" target=\"_blank\" aria-label=\"Link opens in new window (fewer girls in school)\">fewer girls in school<\/a>. They mean delayed diagnoses, broken linkages to care, and <a href=\"https:\/\/www.unaids.org\/en\/resources\/presscentre\/featurestories\/2025\/february\/20250218_us-funding-people-affected-hiv-community-groups\" target=\"_blank\" aria-label=\"Link opens in new window (preventable deaths)\">preventable deaths<\/a>. For key populations at elevated risk of HIV exposure (including LGBTQ+ individuals and the <a href=\"https:\/\/pedaids.org\/2020\/12\/01\/hiv-services-for-factory-workers-in-lesotho\/\" target=\"_blank\" aria-label=\"Link opens in new window (textile workers)\">textile workers<\/a> who produce <a href=\"https:\/\/x.com\/martinplaut\/status\/1897535387347943452\" target=\"_blank\" aria-label=\"Link opens in new window (Trump-branded apparel)\">Trump-branded apparel<\/a>), they mean reduced safety, more discrimination, and poorer health.\u00a0<\/p>\n<p>When Trump speaks of Lesotho, he does so without interest in the staff, supplies, or diplomatic partnerships that U.S. investments have secured.
