\nand correct surname of “XXX<\/u><\/b>“;<\/span><\/span><\/div>\n

\n<\/span><\/span><\/span><\/span>My true surname name is “XXX”<\/b>,
\nas appearing in all my pertinent records including scholastic records and all
\nidentification cards, including my Philippine passport.  <\/span>Copy of my Philippine passport is hereto
\nattached as Annex \u201cB\u201d <\/b>and made an integral part of this Affidavit; <\/span><\/span><\/div>\n
